Lifestyle Medicine National Training Initiative

50% Scholarship Match for Primary Care Providers

The Lifestyle Medicine National Training Initiative (NTI) supports the development of a nationwide lifestyle medicine-trained workforce within Federally Qualified Health Centers (FQHCs).

For every dollar invested in training an eligible primary care provider, ACLM and ABLM will match 50% of the cost (up to $1673.) This matching support helps reduce financial barriers, making it easier than ever for clinicians on the front lines to access this transformative education and training.

In addition to training and certification in lifestyle medicine, NTI participants will gain access to a Health Center Learning Collaborative (HCLC). The HCLC will support their educational and lifestyle medicine clinic-wide implementation journey.

Eligible clinicians who are interested in joining the NTI are asked to submit a formal application.

Application Deadline: Tuesday, September 2, 2025 at 6:00 p.m. ET.

Applications are accepted on a rolling basis. Scholarship recipients must take the certification exam in the year they are awarded.

The scholarship covers 50% of the costs for the required lifestyle medicine certification prerequisites and the certification exam fees:

*The non-refundable exam registration fee ($299 for MDs/DOs and $199 for NPs/PAs) required by the American Board of Lifestyle Medicine (ABLM) is not covered by the scholarship.

ACLM and ABLM will cover up to $1,673 of total $3,347 retail cost.

Eligibility

  • Primary care providers (MDs, DOs, NPs, PAs) currently working in an FQHC

According to the Department of Health and Human Services, to be certified, a FQHC/CHC entity must meet one of these requirements:

  • Is receiving a grant under Section 330 of the Public Health Service (PHS) Act (42 United States Code Section 254a) or is receiving funding from such a grant and meets other requirements
  • Is not receiving a grant under Section 330 of the PHS Act but is determined by the Secretary of the Department of Health & Human Services (HHS) to meet the requirements for receiving such a grant (qualifies as a “FQHC look-alike”) based on the recommendation of the Health Resources and Services Administration
  • Was treated by the Secretary of HHS for purposes of Medicare Part B as a comprehensive Federally-funded health center as of January 1, 1990
  • Is operating as an outpatient health program or facility of a tribe or tribal organization under the Indian Self Determination Act or as an urban Indian organization receiving funds under Title V of the Indian Health Care Improvement Act as of October 1991

What makes the NTI scholarship different from other ACLM scholarships?

NTI scholars can participate in a Health Center Learning Collaborative or HCLC for short, which meets 3 – 4 times a year.
The HCLC is simply a platform to share learnings, challenges, and best practices with health center providers from different regions trying to implement lifestyle medicine and use it to treat the root cause of their patients’ chronic disease.
ACLM is fortunate to have leading clinician members within its community who have implemented lifestyle medicine within different spaces and created adaptable models that can work in many arenas, including the health center space. The learning collaborative will expose NTI scholars to those models and clinicians implementing those models.
